Sports Physicals for the 2013-14 school year will be offered on:
 June 4, 2013 and June 11, 2013
 from 6:00 pm until 8:00 pm
at Wellspan Orthopedics
(25 Monument Rd, Suite 290).  
The cost is $15.  Please take a completed CIPPE packet along with you to the physical.  Forms can be printed from the Forms link on the Athletics page of the district website.
 
If you cannot make either of these dates and need a sports physical, WellSpan Orthopedics and Sports Medicine provides a convenient walk-in option for obtaining a PIAA pre-participation physical evaluation. Student athletes may walk-in to the WellSpan Orthopedic Injury Center during working hours. Wait times may vary based on patient volumes at the center.

PIAA Sports Physical Requirements

A PIAA CIPPE (Comprehensive Initial Pre-Participation Physical Evaluation) form must be completed and turned in to the High School for any student who wants to participate in any sports team for the 2012-2013 calendar year.  The PIAA CIPPE may not be performed earlier than June 1st and shall be effective, regardless of when performed during a school year, until the next May 31st.  The CIPPE form can be picked up at the high school office or printed by clicking on the link below.
 
 

After the student has completed a CIPPE form, should they play other sports in other seasons during the same school year, they must complete a PIAA CIPPE Recertification form.  The CIPPE Recertification form can be picked up at the high school office or printed by clicking on the link below.
